Owner Operator Information
Compansation
| 1. |
Pay - .90 cents per mile, loaded and empty dispatched miles. Fuels Surcharge passed on at 100% of collected surcharges.

Stop/Off pay - $20.00 per stop, exclude first and last stop.
|
| 3. |
100% of loading/unloading pay - if paid by customer and load is delivered on time.
|
| 4. |
IFTA and Single State permit Furnished. Owner must furnish base plates under this plan. |
| 5. |
General Liability, Cargo (200,000), Physical Damage (stated value) and Bobtail insurance deducted by company. You ay provide your own insurance if approved by company.

| 6. |
Bi-weekly pay, two-way settlement available (direct deposit, and EFS-bank card).

| 7. |
Advances availabel. (All advances over $100 per week @ $25.00 per hundred).

Health and Dental insurance available.

Bridge Tolls paid.

Cotractor Liabilities

Escrow fund - $1500.00 - payable at signing. (Financing available)

Occupational Accident Required - unless you have a current certificate.

Pilot Fuel card transactions - n/c per transaction (if team one card per truck).

Fuel and road use tax - $.01 per mile excrowed settled quarterly.

Qualcomm rental - $25.00 per week/ waived if under dispatch 25 days or more per month.

Qualcomm $1000.00 Deposit. (Refunded after one year of service). Deducted @ $100.00 per week.

Scale tickets paid by contractor.

Physical damage insurance available at stated value at 4.5% $1000 deductible.

Passenger insurance premium unavailable at this time.

FedEx service - $25.00 per usage - trip pack to PTL @ no charge. Trip pack to Pride Express Transports, Inc. - $15.00 per pay period. |
AMOUNT DUE WHEN LEASE SIGNED:
If you have current coverage, please bring insurance verification with Pride as loss payee.
1. $1500.00 Escrow, Cash or cashiers check.
2. Base plate.
3. One month insurance permit if taking our insurance.
IT WILL BE OUR INTENT TO COLLECT ALL OF THE ABOVE MONIES UPON EXECUTING THE LEASE AGREEMENT, UNLESS CIRCUMSTANCES DICTATE OTHERSWISE. ALL CONTRACTORS SHOULD BE AWARE OF THE MONIES DUE BEFORE YOU ARRIVE TO PREVENT ANY SURPRISES.
TRACTOR QUALIFICATIONS:
1. 5 years or newer Tandem axle tractor with sleeper - must pass DOT inspection. (Older units considered upon inspection).
2. Current Registration.
3. Title in your name or letter of authorization from legal owner.
DRIVER MUST HAVE BEFORE LEASE:
1. Driver must be 26 years old.
2. Good driving record, 2 years verifiable OTR in 48 states. (WE DO NO NEW YORK CITY).
3. Current CDL-A and Social Security Card.
4. Curretn MVR.
5. Current DOT physical (long form).
6. Voided check to deposit settlements.
7. Registration papers/cab card.
8. Tigle to show ownership.
